Bile acids
- Produced from cholestrol in the liver where they are conjugated with taurine and secreted into the bile and subsequently the intestines . They are then reabsorbed from the ileum into the portal circulation where they are removed by the liver .
- Total serum bile acid levels may be measured by a very sensitive radio immunoassay . An enzymatic spectrophotometry method may be used .
- Overnight fast should be occurred , a pre sample of blood is taken , the patients are then fed a meal and 2 hrs later , a post sample of blood is drawn
- Highest increases in serum bile acids occure in conditions such as photosynthetic shunt , cirrhosis and extra hepatic cholestasis , due to an abnormal portal ciculation or reduced uptake by the liver .
Moderate increases are present in acute hepatitis or hepatic neoplasia , and slight increases may be found with steroid hepatopathy .
